Few companies have formal ways to address human error. When mistakes are made the response is often “They need to pay more attention to their work,” “What's the matter with these people?” or “That's the cost of doing business.”
These can be said in anger, frustration or resignation. Some managers acknowledge that a discussion of lessons learned would be useful, but lament “Who has the time for that?”
