We all make mistakes. Most of them have minor consequences such as forgetting your shopping list or putting on the wrong pair of socks. In fact, human error is behind most quality problems, accidents and near misses.
Errors of judgment can be serious when we drive, operate equipment or climb a ladder. In the office, errors often are caught by ourselves or by a colleague. If they are not identified quickly, the cost or schedule consequences can be serious. In some cases, uncaught errors may pose a risk to safety.
